I Want To Put My Membership On Freeze...

I understand that I am able to put my membership on freeze for a minimum of 1 week & a maximum period of 2 weeks during any 12 week block. I understand that upon freezing my membership I will lose login access completely for the duration of my freeze period. I understand that a 7 day notice is required & this notice period will begin upon submission of this form.
